Parents who own a PeaPod and or PeaPold Plus Travel Bed are being warned to stop using the pop-up travel beds and get a repair kit after various reports that the product can cause suffocation, entrapment and falls.

The U.S. consumer Product Safety Commision (CPSC) and the Libertyville, Il-based manufacturer of the infant bed KidCo Inc. announced a voluntary recall of roughly 220,000 of the infant travel beds.

The product, a small, portable nylon sleep tent designed for infants and toddlers up to 3 years old, is easily folded into a small round shape with a travel bag for easy storage and transport. The device features zippered sides and an inflatable air mattress that is slipped underneath the tent for comfort.

The recall was prompted by reports of the death of an infant and various reports involving children who became entrapped in the device. A year ago, a 5-month-old baby boy was found with his face pressed against the side wall of the tent in New York, New York, the report said. The death has not officially been blamed on the device, the report said.

“Infants and young children can roll off the edge of the inflatable air mattress, become entrapped between the mattress and the fabric sides of the tent, and suffocate,” CPSC said in a press release.

According to the press release, there are six reports of problems with the tent in the United States and three reports in Canada involving infants who were entangled in the device. One incident involved a child who had gotten underneath the mattress that had not been inserted into the zippered pocket on the bottom of the tent.

A defective high pressure scuba diving hose made in Taiwan poses a risk of rupture and possible drowning to divers who use it, according to a release Wednesday by the United States Consumer Product Safety Commission.

So far there have been nine reports of the hoses bursting, enough to warrant an immediate recall this week by the CPSC in cooperation with the companies that imported it: Innovative Scuba concepts Inc., of Colorado Springs, Trident Diving Equipment, of Chatsworth, California and A-Plus Marine of Gulf Breeze, Florida. The manufacturer is Li Chung Plastics Ind. Col Ltd. Of Taiwan.

The dive hoses were sold for about $40 to $50 at scuba diving retailers and online stores across the United States between August 2011 and August 2012, the report said. Stop using these dive hoses immediately and return them to one of the above U.S. companies for a free replacement hose.

The most common cause of death for scuba divers was gas supply failures or problems, emergency ascent and heart failure. According to the Professional Association of Diving Instructors there are roughly 1.6 million to 2.9 million active divers in the United States.  Additionally, every year roughly 150 people die in the U.S. while scuba diving or an estimate one death for every 200,000 dives.

San Luis Obispo, CaliforniaSeven years ago, a bright little eight-year old girl was out trick-or-treating on Halloween night with a group of her friends and family in a residential neighborhood in San Luis Obispo, California.

The group of trick-or-treaters – which included her sisters and a brother — were crossing South Street at the intersection with King Court.  The girl was slower than the pack and lagged behind them as they crossed the dimly-lit street.

It was just before 8:30 p.m. when Annie Tong-Try, 21, of Los Osos was coming down the street in her 1990 Acura Legend and saw the group of about six children up ahead crossing the roadway. However, instead of stopping her vehicle, Tong-Try swerved to the left in an effort to avoid hitting them.

The girl, whose name was Kimberly Macias, was struck by the car. Paramedics responding to the pedestrian accident quickly transported her to French Hospital Medical Center for treatment of critical injuries. Then she was flown by air ambulance to Santa Barbara Cottage Hospitals upon arrival, but sadly the little girl never survived.

California Wrongful Death Lawsuit

Her family was devastated in the aftermath of the crash. They sued the driver, Caltrans and the city for the wrongful death of their child. Although they won the lawsuit and $100,000 from Tong-Try, $75,000 from Caltrans and $35,000 from the city, but this was not about the money. This was a family that clearly only wanted to ensure that such a tragedy would never happen to another child again.

As part of their settlement with the city, the Macias family asked for a memorial bench to be installed at the scene of the accident honoring their daughter and for the city of San Luis Obispo to hand out reflective trick-or-treat bags every Halloween for the next 10 years.

Safety Tips for Children on Halloween

Indeed, Halloween is supposed to be a fun time for children and adults, but one of the biggest threats to their safety is our dark roadways. Typically, most children are not out playing at nighttime or crossing busy roadways.

Sadly children are more than twice as likely to be killed by a car while walking on Halloween than any other night of the year just because it is dark, they are out with friends, adults may not be present, they are excited about getting candy and they are in a hurry to go and get as much as they can before they have to go home.

More than 540 kids under age 14 are killed in pedestrian accidents each year, Safe Kids reports.

Safe Kids analyzed deaths involving children between the years 2002 to 2006. What they learned was 2.2 children on average are killed in pedestrian accidents from 4 to 10 p.m. on Halloween, compared with one child every other evening at the same time.

There are ways you can help make Halloween night safer for children. Using makeup to create a scary face is a better choice than masks that can obstruct a youngster’s view of the roadway. Avoid dark costumes that make it difficult for drivers to see them when crossing the street. Stay on sidewalks, use crosswalks and have an adult walk with younger children.

MONTCLAIR, California — Nicholas Drew Royals, 22, of Ontario was struck and killed while riding his skateboard home from work early Monday morning on Benson Avenue just south of Moreno Street, according to a San Bernardino

The driver of a teal 1995 Nissan Sentra was driving behind Royals  in a southbound direction at about 35 miles per hour when he struck the skateboarder from behind. The driver pulled over and called 911 for police. Royals died at the scene of the pedestrian accident about 4:57 a.m.

In a police interview, the driver told authorities he did not see the skateboard rider who was reportedly wearing dark clothing at the time of the crash. Police say the driver did not appear to be under the influence of alcohol or drugs, but an investigation will be conducted by the Montclair Police Department.

Fatal Skateboard Accidents in the United States

There were 42 skateboard riders killed in 2011 including 30 who were struck by a vehicle or that involved a vehicle in some manner, according to the 2011 USA Skateboarding Fatality Report, a study compiled by Skaters for Public Skateparks, a non-profit skatepark advocacy organization located in Portland, Oregon.

Six of the vehicle-involved fatal crashes were in California, the state with the highest number of deaths totaling 11 in 2011.  Most of the people killed were young males ages 12 to 35 years old, only two victims were females, the report said.

ONTARIO, California — Sergio Becerra, 30, of Bell was killed Monday afternoon when he was struck by a semi tractor trailer truck as he attempted to change a tire on the eastbound side of the Interstate 60 Freeway just west of Euclid Avenue, according to a San Bernardino County Sheriff Coroner report.

The driver of a white 1992 GM truck allegedly struck Becerra about 12:35 p.m. Paramedics responding to the scene of the truck accident pronounced Mr. Becerra dead at the scene of the crash.

California Highway Patrol officers are still investigating the circumstances that led to Becerra’s death, the report said.

The investigation will seek to find out if the driver was under the influence of alcohol, distracted or if there were any circumstances that may have led to the crash that were illegal or reckless.
